This area provides forms and information frequently used by professionals making referrals to us.

• Professionals: Adult social care referrals, resources and training Report a safeguarding concern or incident, request support for a patient, refer for an occupational therapy assessment, and apply for adaptations or DoLS authorisation.
• Professionals: Children's social care referrals How to register an early help assessment, make an early help referral, submit a MARF online or request an OT assessment for a child.
• Professionals: Refer a child missing education Refer a school-age child of who is not registered at a school or being educated in another suitable setting.
• Professionals: Request reduced education provision Request a temporary reduction in education provision if you identify this will meet a pupil's individual need.
