Community Connectors
Community Connectors help Liverpool residents to access services and support in the community, before more formal social care is needed. Our focus is on short term low level support to reduce social isolation, deprivation and falls.
Types of support
Isolation
We can provide information or introduce you to local groups and activities so that you can get out and about and socialise, or connect with others online from the comfort of your home.
Risk of falls
We can facilitate referrals for falls alarms and/or an occupational therapy assessment and give basic advice to reduce risks. We can also provide advice on how to access appropriate exercise groups in the community to build strength.
Deprivation
We can help you access support if you have concerns about your housing situation, benefits and obtaining shopping, health and disability or living environment.
Who can refer
If you, or someone you know, has low level support needs you can refer to the service and a Community Connector will get in touch to help you access the most appropriate services in the community, based on your needs.
If you are a health care worker, GP or other professional and have identified that a patient has low level support needs that could be met by contacting the Community Connectors, you can submit a referral on behalf of the person.
Who cannot refer
Community Connectors are not for people whose needs are more complex. For more complex needs, you must refer to social services for a full assessment of your care and support needs.

What happens next?
When we receive your referral a Community Connector will get in touch within a week to discuss your needs and signpost to the relevant support service.
