Privacy Notice

Your views will be used to assist Liverpool City Council in helping improve testing in schools in the future and to help keep everyone safe during the pandemic.

We are offering a free prize draw with a chance to win one of three £50 Love to Shop vouchers to those who complete the survey. Entry to the prize draw is optional and those who enter will need to provide their name and contact details at the end of the survey. These details will only be used for the purposes of contacting you should you win and will not be passed on to any other organisations. Further details about the prize draw can be found in the Terms and Conditions.

The information you provide when completing the survey will remain anonymous and will be used for analysis purposes only. Should your information be required by other organisations we will obtain explicit and transparent consent from yourselves.

Your information will be stored in a secure location and only accessed by authorised staff with an operational reason to do so.

All identifiable personal information will be destroyed within one month  from the date of the prize draw, however if you wish to withdraw your consent  to use your information prior to this date please contact publichealth@liverpool.gov.uk.

Terms and Conditions

The prize draw is open to parents, guardians or carers of children attending a Liverpool secondary school who complete the schools mass testing survey.

Entrants into the Prize Draw shall be deemed to have accepted these Terms and Conditions.

To enter the Prize Draw you must submit your name, email address and telephone number. No purchase is necessary. If you have any questions about how to enter or in connection with the Prize Draw, please email publichealth@liverpool.gov.uk with 'Schools mass testin' in the subject line.

One entry per email address. Families with more than one child at the school can only enter once. Multiple entries for the same child(ren) or household will be excluded. No participant from the same family or with children from the same household can win more than one prize.

LCC accepts no responsibility for incorrect or incomplete email addresses.

There will be 3 winners for each prize. Winners will be chosen from a random draw of entries received in accordance with these Terms and Conditions.  The draw will take place Wednesday 23rd December 2020. No entrant may win more than one prize on each draw.  

The winner will receive one of three £50 Love 2 Shop vouchers.

LCC accepts no responsibility for any costs associated with the prize and not specifically included in the prize. LCC is not the direct supplier of the prize. 

The winners will be notified by email after the draw and asked for their address so  their prize can be posted to a nominated location. If a winner does not respond to LCC by email within 21 days of being notified by LCC, then the winner's prize will be forfeited and LCC shall be entitled to select another winner in accordance with the process described above will be repeated.   

The name of the winners can be obtained after each draw by sending a stamped addressed envelope to the following address: Schools mass testing survey, Public Health, Liverpool City Council, 1st floor, Cunard Building, Liverpool, L3 1AH

The prize is non-exchangeable and is not redeemable for cash or other prizes.

LCC retains the right to substitute the prize with another prize of similar value in the event the original prize offered is not available.

LCC shall use and take care of any personal information you supply as described in its privacy policy, and in accordance with data protection legislation. Your details will not be shared or sold to third parties. Your personal data may be handled by reputable data processors appointed by LCC.  

LCC accepts no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered by you as a result of entering the Prize Draw or accepting the prize. Nothing in these Terms and Conditions shall exclude the liability of LCC for death or personal injury as a result of its negligence, fraud or fraudulent misrepresentation.

LCC reserves the right at any time to modify or discontinue, temporarily or permanently, this Prize Draw with or without prior notice due to reasons outside its control (including, without limitation, in the case of anticipated, suspected or actual fraud). The decision of LCC in all matters under its control is final and binding and no correspondence will be entered into.

LCC shall not be liable for any failure to comply with its obligations where the failure is caused by something outside its reasonable control. Such circumstances shall include, but not be limited to, weather conditions, fire, strike, industrial dispute, hostilities, political unrest, riots, civil commotion, inevitable accidents, supervening legislation or any other circumstances amounting to force majeure.

The Prize Draw will be governed by English law entrants to the Prize Draw submit to the exclusive jurisdiction of the English courts.
