Make an online housing referral
If you are legally homeless or are at risk of losing your home, you can make a housing referral. You can also use our online form if you are professional working with someone who is homeless or at risk.
We expect you to make every effort to resolve your housing issue, depending on your needs. We've a range of advice on what to do is you are behind on your mortgage, have an eviction notice, or are a care, prison or service leaver in need of housing help. Evidence you need before you start
You will need to provide information that supports what you tell us. This might include:
- proof of identity or immigration status
- an eviction notice
- proof of any medical conditions
- a GP’s letter that proves you are pregnant
- your last 5-year address history
- contact details of the person asking you to leave the property

If you have done everything to try to resolve your housing situation and you qualify for help, you can make a housing needs referral. You can also use this form to refer someone you are concerned about.
To complete this form you will need an email address. If you do not have an email or need help finding any evidence requested in this form, please call 0800 731 6844 between the hours of 9am and 4.30pm.

What happens next?
Once you have submitted your housing needs assessment request, you will receive an email with a reference number.
If you have a submitted all the evidence the form asks for, we will be in touch in XX days. If you still need to upload evidence to support your referral, you will have 10 days to send it to us.
