Care leavers – your health and wellbeing

Find out how to access a range of health, sexual health and substance misuse services, support for young parents and a free Lifestyles leisure centre membership.

Get a free Lifestyles membership

As a care leaver you can join any Lifestyles Centre in Liverpool free of charge. We will need to process your application, so please contact us for more information.

Access a specialist nurse

Between the ages of 16-17 years, you will be allocated to a Children in Care specialist nurse who will complete annual health assessments with you. Between the ages of 18-25, the specialist nurse team offers an extended advice and support service.

Get your Health Passport

Before your 18th birthday, you will be given a Health Passport. It will detail your birth history, immunisation and health history, and list helpful contacts.

The Health Passport can be sent to you in paper or sent electronically. Your PA can also save a copy of it on your file if you are in agree. If you have not received a copy of your Health Passport let your PA know and they will make sure you receive it.

Register with local health practices

You can ask your PA to help you register with a GP, dentist or optician close to where you live. If you need to travel to appointments until you are registered locally, we will pay your transport costs.

Sexual health support

Sex is a natural part of life, and you should be able to enjoy a positive, happy and healthy relationship when you are ready. Support and advice regarding sexual health.

Axess 4 U for young people aged under 19 years - offers contraception, emergency contraception, STI information and testing, advice and guidance and pregnancy testing.

Axess for young people aged over 19 - provides contraception and sexual health advice. To make an appointment with Axess at any of their clinics based at the addresses below call 0300 323 1300.

The Butterfly Clinic - provides sexual health support for trans and non-binary young people.

Help with substance misuse

River Young People Support Service (changegrowlive.org) is a free and confidential service for children and young people under 25 in Liverpool who are affected by their own, or someone else’s, drug or alcohol use. 

You can refer yourself or ask someone to refer you (changegrowlive.org). This service also supports parents and professionals.

Support for young parents

Family Nurse Partnership (FNP) is a free and voluntary programme for under 20s expecting their first baby. The family nurse will visit you every week from early in your pregnancy until your child is two years old. Dads are welcome to join in during home visits.

You can refer yourself to the FNP or ask your PA to make a referral for you.

Cycle of Life Programme

This programme helps young people seeking asylum:

  • learn to ride a bike and maintain one
  • navigate travel routes around the city use travel apps to explore the region

You'll benefit from the course by developing the skills to travel while building new friendships. Speak to your PA to find out more.
