Request targeted family help
If you have identified a child and their family with increasingly more complex needs who would benefit from targeted interventions from a Family Help outreach support worker or our Family Ties Reimagined team, you can make a family help referral.
How to make a Level 3 referral
All organisations should refer the child and family to the Family Help Teams for support using our family help referral form.
- Step 1: Obtain consent from the parent carer. If consent to a referral is denied when you are concerned about a child at Level 3 please get advice from a Family Help Team.
- Step 2: Complete the family help referral form below. This form should also be used to refer a family for support from our Family Ties Reimagined team.

- Step 3: Your referral will be triaged by a consultant social worker.
- Step 4: The consultant social worker or team manager who reviews your family help referral form will email you about the outcome.
Things to consider when making a family help referral
Family Help is consent-based family support. Our teams work alongside children and their families to undertake the Family Help assessment and agree a Family Help plan. Our support workers act as the lead professional coordinating a multi-agency package of targeted family support.
