Lost your paper licence?

Your paper licence is a legal document issued by Liverpool City Council and it confirms the details of your licence.

If you have lost your paper licence, you will need to apply for a duplicate one using the form below. Please select 'duplicate licence/replacement badge' when prompted.

You will need to upload images of:

  • your statutory declaration (see information below) and
  • your current driving licence.

A statutory declaration is a formal statement affirming that something is true, signed in the presence of an authorised official like a solicitor or notary public.

It's a legally binding document used to confirm facts, especially when official documents are not available.
