How to make an approved agency early help referral

Organisations that work directly with adults or an approved referral agency - for example GPs, North West Ambulance Service, probation, adult mental health services - can complete an online early help referral.

• Step 1: Obtain consent from the parent carer - you cannot submit a referral without first gaining consent.
• Step 2: Complete the early help referral form below.

• Step 3: Your referral will be triaged by a consultant social worker. The family may be signposted to services, or a professional may be identified to complete an Early Help Assessment.
• Step 4: The consultant social worker or team manager who reviews your early help referral form will email you about the outcome.
