Request targeted family help

If you are working with a child and their family who have complex needs and you think they would benefit from targeted family help, or support from our Family Ties Reimagined team, you can make a referral using the online form on this page.

How to make a referral

• Step 1: Obtain consent from the parent carer. If consent to a referral is denied when you are concerned about a child at Level 3 please get advice from a Family Help Team.
• Step 2: Complete the referral form below. This form should also be used to refer a family for support from our Family Ties Reimagined team.

What happens next?

Your referral will be triaged by a consultant social worker who reviews your referral and will email you about the outcome.
