Changes to your circumstances or care needs

If you receive support from us and your needs change, there has been a change to your finances or you cannot afford to pay for your care, you must tell us so we can review your care package. 

Changes may also include if you are going on holiday or in hospital and do not need your home care package for a short period.

Change of address or leaving Liverpool

If you receive support services from us, you can report a change of address using the general enquiry form.

Change in care needs

If you receive support services from us, you must tell us if your needs have changed so we can review your care package.

You can submit a support enquiry to tell us about changes to your care needs.

Change in finances

If you pay towards the cost of your care, but your financial circumstances have changed, you must tell us so we can review how much you pay. You can request a reassessment at any time by completing a payment enquiry.

Going on holiday

If you don't need support for a short period of time, for example, if you are going on holiday or staying with family, you must contact your care provider to let them know.

You must normally give 48 hours' notice or your care provider may still charge for the service.

If your holiday goes over 2 weeks, the package of care will be automatically cancelled and a new assessment will be required.

Hospital admission

If a service user is in hospital you, or someone acting on your behalf, must contact your care provider and allocated social worker.

If the hospital admission goes over 2 weeks, the package of care will automatically be cancelled and a new assessment will be needed. This will normally be done by the hospital discharge planning team.

Service user has passed away

Tell Us Once service

Tell Us Once is an optional free service that lets you report a death to most government organisations and council departments in one go.

When you register a death at Liverpool Register Office, the registrar will ask if you want to use the Tell Us Once service. This means you won't need to get in touch with individual organisations or send copies of death certificates. Find out more about Tell Us Once.

Inform adult social care

If you decide not to use the Tell Us Once service, you can use the general enquiry form to notify us of the death, if the deceased was receiving support services from us.

We will then notify any social care and charging teams to suspend payments, if the service user was paying towards the cost of their care.

The social care charging team will let you know if there are any outstanding charges on the service user's account.
