• Request support for an adult Request support for an adult under the Care Act.
• Occupational therapy referrals If you are a therapist that can prescribe core stock, bathing equipment and/or minor adaptations or a GP, health care worker or other professional, you can make a referral to OT.
• Mersey Care bathing adaptations referral Mersey Care occupational therapists can refer direct to Agency Services if you have previously identified bathing equipment does not meet your client's needs.
• Minor adaptation or key safe request If you are a health and social care practitioner and your client needs minor adaptations to remain safe and independent at home, you can submit a request to us.
• Approved Mental Health Professional Service (AMHP) The AMHP service is responsible for undertaking assessments under the Mental Health Act in Liverpool, and in some cases outside the city.
• Apply for DoLS authorisation How to apply for DoLS authorisation from Liverpool City Council if you are a managing authority (hospital or care home).
