Carer's assessment
If you look after someone who couldn't manage without your support, you are entitled to a carer's assessment.
You are a carer if you give regular care and support to someone else and do not get paid for doing so. You might help with:
- washing, dressing or taking medicines
- getting out and about and travelling to medical appointments
- shopping, laundry and cleaning
- paying bills and organising finances
- You may also give other support like keeping them company or watching over someone who can't be left alone.
An assessment will give you a chance to think about what support you need and how your caring responsibilities affect your wellbeing and quality of life.
It will look at how we can help make your life easier. You might want to think about:
- how you feel about being a carer
- if you get enough time to yourself
- if you get enough sleep
- are your relationships with family or friends affected
- is the person you look after getting the right help or enough help
- any other worries you may have
- if it affects your work and finances
How do I get assessed?
If you are aged 18 or above and care for someone who is a Liverpool resident, you can complete the assessment online below.

Before you start
Before you start the form, you'll need:
- your NHS number (if known)
- your GP's name, address and phone number
- details of the person you care for, including their date of birth and NHS number (if you have it)
- medical and support details for the person you care for
The form should take 10 to 15 minutes to complete.
Complete an assessment online
You must have an email address and register for an adult social care online account to complete an assessment.
- As part of the registration process, we will send a security authentication code to your email address.
- Once registered, you can complete and submit the assessment form, or save and come back to it.
- Each time you sign in to your account, we will email you a different security authentication code.

What happens next?
We'll contact you and:
- work with you on a support plan.
- help you to stay healthy.
- give advice on education or carers’ training.
- take you through your rights if you are in paid employment.
- make sure you’re getting the right financial support.
- help you to have a break.
- respect you and your caring role.
Will I have to pay for the services I receive?
There may be a cost for some of the services we provide. If this is the case we will work out how much you, or the person for whom you care, can afford to pay. Find out more about paying for care.
