If you are a care provider, GP, clinician or other professional and have identified that an adult with care and support needs is at risk of harm, you must report it.

If you have identified that an adult over 18 finds it difficult to manage day-to-day activities at home due to a physical, mental or sensory impairment, illness or old age, you can request a referral to OT if you are:

  • a therapist that can prescribe core stock, bathing equipment and/or minor adaptations
  • a GP, health care worker or other professional

If you are a Mersey Care occupational therapist and have previously identified bathing equipment which does not meet your clients' needs, you can refer direct to Agency Services for a Disabled Facilities Grant for a level access shower.

If you are a health or social care practitioner and have identified, through an assessment, that your client requires minor adaptations to help them stay safe and independent at home, you can submit a request online for the council to complete the adaptations.

Once the managing authority (care home or hospital) has determined if an urgent or standard authorisation is required you must complete the relevant forms.

The AMHP service is responsible for undertaking assessments in Liverpool. Before you request a referral you must ensure that you - or another professional worker - have seen and spoken with the person within the previous 24 hours.
